Every winter, hemorrhoid patients suffer. This is because, if the weather gets cold, blood vessels shrink and blood circulation becomes obstructed. As well, patients may have to frequently drink alcohol at the company meeting after work, which makes the symptom worse. According to 2015 major surgery statistics reported by the National Health Insurance Service, approximately 60,000 cases of hemorrhoid surgery were performed in January, February and December, accounting for 30% of cases (about 197,000 cases) per year.

 

Hemorrhoid refers to anal diseases such as piles, anal fistula, and anal fissure, but the hemorrhoids that we generally think of means ‘piles’. The hemorrhoid occurs when blood vessels under the mucous membranes forming the anus are stretched and the blood accumulates in them. It is divided into an external hemorrhoid and internal hemorrhoid. The external hemorrhoid is a swelling of tissue outside the anus and the internal hemorrhoid is that tissues inside the rectum exit the anus.

 

In general, the hemorrhoid causes itching, pain, discomfort, bowel movement disorder, and so on. However, rapid treatment is needed if it leads to bleeding, proctoptosis, or infection because the symptoms can develop into the anal abscess or sepsis. There are two major treatment methods; 1) conservative treatments as a non-surgical method, and 2) surgical method in order to treat the disease.

 

Different kinds of methods in conservative treatment are performed according to the degree of symptoms and ‘sitz-bath with warm water’ is one of the most commonly used methods. The sitz-bath relaxes the anal sphincter, which is the main cause of the pain, in order to reduce the pain. The effect of sitz-bath with warm water is very significant in the beginning stage of hemorrhoid. However, if it is exacerbated to the second stage of the internal hemorrhoid, a "rubber band ligation" may be performed in which the tissue falls off by fixing the roots of the elongated hemorrhoid with a band so that blood does not pass. However, this method can not be used in case of the external hemorrhoid because of the pain.

 

On the other hand, the most common surgical procedure known as 'hemorrhoid surgery' is divided into hemorrhoidectomy and hemorrhoid surgery using a procedure for prolapse and hemorrhoids. hemorrhoidectomy is usually performed for patients with stage 3 or more, in case of hemorrhagic internal hemorrhoid which is not improved by conservative treatment, or in the case of the external hemorrhoid with a large lesion and severe pain. Pain relief and regular sitz-bath are needed to relieve pain for about 4 to 6 weeks after the surgery, and it takes an average of 2 to 3 weeks to return to daily life.

 

Surgery using the procedure for prolapse and hemorrhoids is an operation method that pulls up the elongated anal mucosa and the hemorrhoidal tissue to return to the original anatomical position and reduce the blood flow of the external hemorrhoid. Compared with the hemorrhoidectomy, the wound and pain are fewer, and the operation time is short and the recovery period (about 7 days) is faster. However, this method is not necessarily applied all the time.

 

Professor Park, Min Geun of the General Surgery, CKU International St. Mary’s Hospital, said "Hemorrhoid surgery the procedure for prolapse and hemorrhoids is usually performed when it is diagnosed as 2 to 3 stage of hemorrhoids. However, sometimes the surgery may not be possible if the external hemorrhoid is too severe or the anus is narrow."

 

Despite the development of treatment methods, hemorrhoids are thought to be one of the diseases that many people still want to hide because of their shame on the lesion and the fear of surgery.

 

Professor Park, Min Geun said, "Just as a lot of sunshine causes wrinkles on the face, the hemorrhoids are naturally occurring anal diseases if they have bad bowel habits." If you are suspected of hemorrhoids, it is good to take medical care and prevent it," he said.
